Request for Proposals (RFP) Installation of One-Way Gravel Drive for Pre-K Drop-Off/Pick-Up Lane Mountainburg Public Schools 1. Introduction Mountainburg Public Schools (the “School”) is seeking proposals from qualified contractors for the installation of a one-way gravel drive for a pre-kindergarten (Pre-K) drop-off/pick-up lane at its facility located at 2015 Lake Fort Smith Road, Mountainburg, AR. 2. Project Description The project involves the installation of a new one-way gravel driveway designated for Pre-K student drop-off and pick-up. The proposed drive will consist of the following: • Site prep, • Build road, base dirt provided, • Gravel surface with proper drainage system to prevent water pooling, including culverts as needed, • Permits required by City of Mountainburg or ARDOT, • Removal and disposal of waste, • Location of utilities and authorization to dig 3. Submission Requirements Proposals must be submitted in a sealed envelope clearly marked "Pre-K Driveway Installation RFP" and addressed to: Mountainburg Public Schools 129 Hwy 71 SW Mountainburg, AR 72946 Pre-K Drive RFP – Attn Dr. Debbie Atwell Proposals must be received by July 22, 2024 at 1:00 PM. Proposals must include the following information: • Company name, contact information, and license number. • Proof of general liability and worker's compensation insurance, and eligibility to provide services and contract with state entity. • Minimum of three references for similar projects. • Detailed project plan outlining the scope of work, materials to be used, construction timeline, and a breakdown of all associated costs. • Warranty information for materials and workmanship. 4. Selection Criteria Proposals will be evaluated based on the following criteria: • Qualifications and experience of the contractor. • Completeness and clarity of the proposal. • Proposed materials and construction methods. • Project timeline. • Cost effectiveness. • References. The School reserves the right to reject any or all proposals and to waive any irregularities in the submission process. 5. Site Visit A pre-bid meeting and site visit will be held on July 16 at 3:00 at the School address provided above. Attendance at the pre-bid meeting is highly encouraged but not mandatory. 6. Mark Your Calendars! Attending campus open house and beginning of the year meetings is one of the best ways to start the school year.

Bids are still be taken for HVAC units for the MMS Gym and Carl D. Rogers Gymnasium.

ATHLETIC PHYSICALS: It is time to start thinking about physicals and your child's DragonFly for next season. Students who plan on participating in grades 7th - 12th athletics for the 2024-2025 school year will need an updated physical before he/she can participate. Physicals are good for one year from the date you receive it. For example, if your child's physical was in August 2023, his/her physical is good until August 2024. You can get a physical for your child a couple ways: 1. Receive a physical from your child's pediatrician. If you choose to go this route, please print both the following forms, parents fill out the Health History Form and take the Physical Form to . These are the physical papers the AAA would like for us to use. 2. The Mountainburg Family Clinic has agreed to do a physical for $25. This is very cheap compared to most. It may even be more economical than some co-pays to pay the $25. The clinic should have the form, so you will not need the above form for the clinic. New athletes or incoming 7th grade athletes, you will need to create a DragonFly account and complete the required items. Existing 8th - 12th athletes, you will need a physical and an update to your DragonFly account. Everyone has to sign off on the Sports Medicine and AAA Participation Agreement every year. It is always awesome to hear about the accomplishments of MHS graduates. Below is a release from the University of Arkansas - Fayetteville about Sydney Taylor, MHS Class of 2021. Along with this achievement, Sydney has chosen to pursue her Master's degree. Congratulations Sydney! We are blessed to have wonderful role models for our students to look up to and see that hard work pays off.
